Automatic Meal Reservation Form

Please complete this form (one per family) to make your automatic family night meal reservation. Automatic reservations may also be called in (324-2130).

If you fill out this form, you will be added to the meal list each week and you won’t have to sign up for any meals for the rest of the season. If you will be gone on a certain date, notify the church office by 4:00pm on the Monday before the meal!

PLEASE USE THIS FORM ONLY IF YOU WOULD LIKE TO RESERVE YOUR MEAL AUTOMATICALLY EACH WEEK.  OTHERWISE, PLEASE USE THE WEEKLY SIGN UP FORM OR CALL THE CHURCH OFFICE TO MAKE YOUR RESERVATION.
